Before you start

You need:

a valid Yukon Health Care Insurance Plan number; and
documentation to support your requested change. You’ll be asked to upload these with your application if you update the following.
- Given name
  - 1 copy of your primary ID; and
  - name change document.
- Last name
  - 1 copy of your primary ID; and
  - Additional document depending on the reason:
    - Legal change: name change document
    - Marriage: marriage license
    - Divorce: no additional document required
- First Nation status number
  - 1 copy of your First Nation status card.

Primary ID accepted: birth certificate, passport, citizenship card or permanent resident status
File formats accepted: .png, .jpeg or .pdf.
Maximum file size: 10 MB
